Eligibility at a Glance
Need to refer a patient for transportation to medical care? Here’s a quick summary of who qualifies and what documentation we’ll need to expedite the process.
Helping the Patients Who Need It Most
Who We Help and How to Qualify
We assist patients who:
Are traveling for medically necessary care or clinical trials
Cannot afford the cost of travel on thier own
Are able to travel without medical supervision (with or without a caregiver)
Are traveling 50+ miles (ground) or 100+ miles (air)
Appointments are scheduled within the next 45 days
Are physically able to travel independently or with one companion
Have household income at or below 300% of the Federal Poverty Level

Required Documentation
Have These Documents Ready Before You Start
To process a request, we must confirm financial need and medical appointment details.
Option A
Current Year Tax Return — all pages
Option B
Social Security or Disability Benefit Statement and your last three months of bank statements
Option C
Social Security or Disability Benefit Statement
& IRS Non-Filing Letter
We do not accept pay stubs or W-2s alone as proof. Additional documentation may be requested.
Additional Guidelines to Keep in Mind
Requests must be submitted at least 10 days before the appointment
Rideshare is only available in select Virginia cities (Virginia Beach, Norfolk, Chesapeake, Portsmouth, and Suffolk)
For longer distances, patients may qualify for volunteer pilot flights or commercial travel assistance
