Do you provide rides?
No. We do not provide rides to appointments.
Do you assist with local, non-medical trips (i.e. grocery store, pharmacy, work)?
No. We do not assist with local, non-medical trips.
Do you assist with trips to the Community Recreation Centers or a Gym?
No. We do not assist with travel to the Recreation Centers or Gyms.
Do I have to submit a new application every month?
Yes. Approval is not guaranteed and is based on available funding each month.
Can I call in to complete an application over the phone?
No. You cannot fill out applications over the phone. You must apply through our website using a computer, smartphone, or tablet.
Is there an alternative way to apply for assistance besides the online application for those with no access to internet?
No. However, we can accept an application on your behalf from a social worker, friend or family member.
Are the gas cards reloadable?
No. The gas cards are not reloadable. A new card will be mailed out each month you are approved.
Do they have to be used at a specific gas station?
No. The gas cards can be used at any gas station however you cannot pay at the pump, you will have to go inside to use the card.
What is the minimum mileage requirement to qualify for ground assistance?
The minimum mileage is 50 miles total.
What if I can’t fly but I’m traveling over 300 miles and need fuel assistance?
You may apply to the program and have your doctor fax us a letter stating that it is medically necessary for you to drive to treatment due to your medical diagnosis. We will review once received.
Do you provide local bus passes for public transit?
We do not assist with local bus passes, subway, or light rail train.
Can the gas cards be used for Uber, Lyft or Taxi’s?
No. Gas cards are marked for fuel only.
If I don't have a vehicle, do you provide a bus or train ticket?
Yes, based on the distance you are traveling and it is on option on the application.
Can you help pay for my rental car, toll fees or lodging?
We cannot assist with rental cars, toll fees, parking fees or lodging. For a comprehensive list of resources, please feel free to visit www.findhelp.org.
What are the income guidelines?
Our current income guidelines are based on total household income and family size who are below 300% of Federal Poverty guideline.
Will I receive the gas card before my appointment?
Depending on when you apply in the month and the processing of the application, we cannot guarantee it will arrive before your scheduled appointment.
How will I know when to expect the gas card?
We cannot track any gas card sent, but you can sign up for informed delivery through USPS. It helps give you a better idea of when your mail will arrive.
Can I receive reimbursement for past medical travel?
We are only able to assist with the appointments within the month of your application. We do not back date to previous months.
How do you calculate how much I will receive?
The amount on the gas card is determined by the miles travelled.
