Information After Applying

Please carefully note the following Mercy Medical Angels' guidelines for commercial travel.

 

Mercy Medical Angels (MMA) may sponsor a round-trip flight for an eligible patient and one non- medical attendant (if the doctor deems it medically necessary) on a commercial flight, coach seating. The patient must be able to sit on their own accord once they are in the seat requiring no medical attention. Nonstop flights are not guaranteed, nor is an airline. 


Please note: There is an administrative processing fee for each ticketed passenger, based on income. Flights from Hawaii and Alaska will also be assessed a surcharge of $150 per ticket if you fall under 300% of the Federal Poverty Line. A surcharge of $200 will be assessed for those with income above 300% of the Federal Poverty Line. Puerto Rico will also be assessed a surcharge of $100 per ticket if you fall under 300% of the Federal Poverty Line. A surcharge of $150 will be assessed for those with income above 300% of the Federal Poverty Line. Upon receipt of your financial information, MMA will confirm your precise fee per ticket. The fee will be collected via credit or debit card prior to booking.

 

Please expedite the following documentation: ** All documentation is required 10 business days before departure to move forward with your flight request. **

  • Medical Clearance Form - MMA requires the completion of Mercy Medical Angels' medical clearance form from the doctor who has most recently seen the patient and is aware of their travel request for medical treatment/appointment. Said form will be faxed directly to the doctor from MMA upon receipt of your application. Please ensure the fax number of your releasing physician is correct.
  • Appointment Verification - Please provide a formal confirmation from your treating facility with scheduled appointment date(s) and time. This can be faxed or emailed. For some facilities, you may access via My Chart and a screenshot or photo will suffice. ** Appointment verifications must have the patient’s name on them**
  • Income verification (Gross) - MMA requires the following documents for the total household/everyone living in the home: 2023 tax return(s) including schedule 1/C/D/E showing gross income (if filed), 2023 Social Security award letters, pension, IRAs, stocks, etc. Mercy Medical Angels is a need-based program, and the above is required to assess eligibility. *If you provide a Social Security letter you will need 2 months of bank statements or an IRS non-filing letter to go with it.*
  • IRS transcript of filing or non-filing letter

  • You will have to create a login and provide them with your tax information (if filed) and some identifying information to obtain your transcript or letter. When you receive your copy, please email the email below.
  • DD214- Unredacted form and/or active-duty paystub if applicable.

 

Tickets cannot be changed without penalty, and MMA is not responsible for change fees and/or increase in fares due to the patient's rescheduling of flights. Baggage fees are patient responsibility.


Processing fees are non-refundable.


Patients must be able to transfer out of a wheelchair into their seat on the aircraft with little to no assistance (they cannot have someone literally pick them up and place them in the seat). If the patient requires oxygen, some airlines may provide it during the flight, or the patient must have a portable oxygen concentrator (POC) approved by the FAA. The commercial airlines require no less than 48 hours advance notice to prepare for the equipment needed. The patient would be responsible for making all arrangements and paying any fees associated with the oxygen required for travel. Please consult your physician if necessary.


Beginning May 7, 2025, every air traveler 18 years of age and older will need a REAL ID-compliant driver's license, state-issued enhanced driver's license, or another acceptable form of ID to fly within the United States. 

 

Rideshare/Uber Guidelines: 

This is a financial need-based program that provides round trip Uber assistance from the Airport/FBO to the facility (hospital/treatment center). Please see below following guidelines:

  • It will be in the form of a voucher that you can upload to your Uber account.
  • It is limited to $60 and will only cover a maximum of $30 one way. 
  • This voucher will be emailed to you and can be used for round trip only, we will not be able to further assist you for more than 2 trips.
  • It is limited to use of a regular sized car and only valid for your travel dates.
  • Tips given will come out of your Uber account and will not be covered by the voucher.
  • Any extra charges to your account will not be refunded by our organization and you will need to speak with Uber directly regarding this.
